Urological Cancers

Urological cancers are a group of cancers that affect the organs of the urinary system and the male reproductive system. These include cancers of the prostate, bladder, kidneys, testes, penis, and upper urinary tract. Collectively, they are among the most common cancers diagnosed worldwide, particularly in men.

What are Urological Cancers?

  • Kidney cancer – usually develops within the kidney tissue itself.

  • Upper tract urothelial cancer (UTUC) – affects the lining of the kidneys and ureters.

What are the Symptoms of Urological Cancers?

Symptoms vary depending on the organ involved. Many early-stage cancers cause few or no symptoms, which is why routine screening and prompt investigation of concerning signs are important.

Common symptoms may include:

  • Blood in the urine (haematuria)
  • Difficulty passing urine
  • Frequent urination, especially at night
  • Pain or burning during urination
  • A weak or interrupted urinary stream
  • Pelvic pain
  • Persistent lower back or flank pain
  • A lump or swelling in the testicle
  • Blood in the semen
  • Erectile dysfunction
  • Unexplained weight loss
  • Fatigue
  • Bone pain in advanced prostate cancer

Any persistent urinary symptoms or visible blood in the urine should always be assessed by a urologist.

What Causes Urological Cancers?

While the exact cause is often unknown, several factors can increase the likelihood of developing urological cancers.

These risk factors include:

  • Increasing age
  • Smoking
  • Family history of prostate, bladder or kidney cancer
  • Certain inherited genetic conditions
  • Exposure to industrial chemicals
  • Long-term bladder irritation or chronic infections
  • Obesity
  • High blood pressure
  • Previous radiation therapy
  • Human papillomavirus (HPV) infection, particularly in penile cancer
  • Undescended testicle, which increases the risk of testicular cancer

Having one or more risk factors does not necessarily mean someone will develop cancer.

How are Urological Cancers Diagnosed?

Diagnosis begins with a detailed medical history and physical examination, followed by investigations tailored to the suspected condition.

These may include:

  • Urine analysis and urine cytology
  • Blood tests, including prostate-specific antigen (PSA) testing
  • Ultrasound scans
  • CT scans
  • MRI scans
  • PET scans in selected cases
  • Flexible cystoscopy to examine the bladder
  • Prostate biopsy, including MRI fusion biopsy where appropriate
  • Kidney biopsy in selected cases
  • Testicular ultrasound
  • Bone scans for advanced prostate cancer
  • Tissue biopsy to confirm the diagnosis

Once cancer has been confirmed, additional imaging may be performed to determine whether it has spread and to establish its stage.

What are the Treatment Options for Urological Cancers?

Treatment depends on the type of cancer, its stage, overall health, and individual patient preferences. A multidisciplinary team works together to develop the most appropriate treatment plan.

Treatment options may include:

  • Active surveillance for selected low-risk cancers
  • Robotic-assisted surgery
  • Open surgery where appropriate
  • Partial or radical nephrectomy for kidney cancer
  • Radical prostatectomy for prostate cancer
  • Radical or simple cystectomy for bladder cancer
  • Transurethral resection of bladder tumours (TURBT)
  • Chemotherapy
  • Immunotherapy
  • Targeted therapy
  • Hormone therapy for prostate cancer
  • Radiotherapy
  • Intravesical treatments for certain bladder cancers
  • Clinical trials where appropriate
